Premature Ovarian Failure In Traditional Chinese Medicine Perspective

Premature ovarian failure is defined as a condition of which the ovarian is no longer responded to the brain in production of follicle before the age of 40, leading to high levels of FSH in the bloodstream and menopause hormonal symptoms. It is caused by either the accelerated loss of eggs or follicles are become less responsive to hormone stimulation from the pituitary gland. It has been estimated that POF affects 1% of the population. While conventional medicine uses synthesis medication to stimulate the ovarian response and artificial insemination with only 5% success rate accompanying with a high medical cost, it drives many women to seek help from traditional medicine, including the traditional Chinese medicine.

Since each woman is diagnosed as her own entity, some symptoms may be found in some women, but not in the other, depending to the diagnosis.
I. Causes of POF in TCM perspective
A. Excessive yang
Caused by an excessive loss of vital essence and nutrient fluid, leading to dryness of skin, chapped lips, nose bleed, mouth sore, urinary burning, etc. and interfering with the kidney function in hormonal regulation, resulting in ovarian abnormal function in responding to the hormone secretion from pituitary gland.
B. Spleen qi deficiency

1. Diet modification

vi) Coffee
Intake of large amount of coffee daily to fight off stress may have a negative effect in the reproductive system. As study show that women who consume 300 mg or more of caffeine take longer to conceive than women who do not or take less.
vii) Drug and alcohol
Drugs and alcohol can influence an imbalance of reproductive hormones. As drug increases the tension of the nervous system leading to hormone imbalance, excessive alcohol drinking can cause liver damage, which abnormal function of liver in all kinds of metabolism, causing blood and qi stagnation in the liver and effecting a couple's reproductive abilities. Alcohol also causes nervous tension, leading to over production of certain hormones, thereby, increasing the risk of infertility.
viii) Carbonate Soda
Carbonate soda contains high amount of sweetener, caffeine and phosphate which can interfere with calcium absorption as well as stimulating the over production of certain hormone including cortisol, leading to fatigue, stress and anxiety.
ix) Most animal and dairy products
Because most animal and diary products are considered acidic which can distort the balance of pH levels in the body, making cervical mucus hostile to sperm invasion as well as preventing the implantation of egg.
2. Chinese herbs
a) Herbs for Blood Deficiency
i. He shou wu (fleeceflower root)
He shou wu is a liver tonic herb, it promotes liver function in blood formation and increase the kidney qi in assisting the liver in transporting blood to the heart for circulation, thereby reducing the blood stagnation causes of deficiency. It also improves the kidney in urinary secretion, thus decreasing the risk of blood stagnation caused by fluid retention.

Estrogen and progestin bioactivity of foods, herbs, and spices

In the study of the content and bioactivity of plant (phyto) estrogens and progestins in various foods, herbs, and spices, before and after human consumption, over 150 herbs traditionally used by herbalists for treating a variety of health problems were extracted and tested for their relative capacity to compete with estradiol and progesterone binding to intracellular receptors for progesterone (PR) and estradiol (ER) in intact human breast cancer cells, posted in PubMed, indicated that the six highest ER-binding herbs that are commonly consumed were soy, licorice, red clover, thyme, tumeric, hops, and verbena. The six highest PR-binding herbs and spices commonly consumed were oregano, verbena, tumeric, thyme, red clover and damiana. Some of the herbs and spices found to contain high phytoestrogens and phytoprogestins were further tested for bioactivity based on their ability to regulate cell growth rate in ER (+) and ER (-) breast cancer cell lines and to induce or inhibit the synthesis of alkaline phosphatase, an end product of progesterone action, in PR (+) cells. In general, we found that ER-binding herbal extracts were agonists, much like estradiol, whereas PR-binding extracts, were neutral or antagonists. The bioavailability of phytoestrogens and phytoprogestins in vivo were studied by quantitating the ER-binding and PR-binding capacity of saliva following consumption of soy milk, exogenous progesterone, medroxyprogesterone acetate, or wild mexican yam products containing diosgenin. Soy milk caused a dramatic increase in saliva ER-binding components without a concomitant rise in estradiol. Consumption of PR-binding herbs increased the progestin activity of saliva, but there were marked differences in bioactivity. In summary, we have demonstrated that many of the commonly consumed foods, herbs, and spices contain phytoestrogens and phytoprogestins that act as agonists and antagonists in vivo.

Estrogenic activity of natural and synthetic estrogens in human breast cancer cells in culture

In the study to investigate the estrogenic activity of various environmental pollutants (xenobiotics), in particular the xenoestrogen o,p-DDT, and compared their effects with those of endogenous estrogens, phytoestrogens, and mycoestrogens on estrogen receptor binding capacity, induction of estrogen end products, and activation of cell proliferation in estrogen-sensitive human breast cancer cells in monolayer culture, posted in PubMed, indicated that natural endogenous estrogens, phytoestrogens, mycoestrogens, and xenoestrogens bind estrogen receptor (ER) in intact cells, but demonstrate marked differences in their ability to induce end products of estrogen action and to regulate cell proliferation. All of the different classes of estrogens stimulated cell proliferation at concentrations that half-saturated ER, but only some classes were able to induce estrogen-regulated end products. Genistein, a common phytoestrogen found in soy foods, differed from the xenoestrogen DDT in its effects on cell proliferation and ability to induce estrogen-regulated end products. Moreover, we found that many of the foods, herbs, and spices commonly consumed by humans contain significant amounts of phytoestrogens, and consumption of soy milk, a phytoestrogen-rich food, markedly increases the levels of phytoestrogens in saliva. In conclusion, our in vitro results predict that a diet high in phytoestrogens would significantly reduce the binding of weak xenoestrogens to ER in target tissues in vivo.

Follicular cysts

Follicular cyst, is a sac filled with fluid, normally forms at the time of ovulation as a result of mature follicle has become involution or when ovulation does not occur. That means there is a follicle which doesn't rupture or release its egg but instead grows in the ovaries and becomes a cyst. During every month of menstrual cycle About one-fourth of women with this type of cyst experience pain when ovulation occurs,the follicular cyst may rupture, causing severe pain to about one-fourth of women.

Symptoms
1. In most case there is no symptom at all

Unilateral oophorectomy is performed in the hospital with general anesthesia, In the surgery, a laparoscopic, thin tube containing a tiny lens and light that inserts through a small incision in the navel with a camera on the other end to allow your doctor to see the abdominal cavity on a video monitor. After the ovary is detached, it is removed though a small incision at the top of the vagina.
b.1. Vertical incisions
Vertical incisions give the doctor better view of of the abdominal cavity but it will leave some notable scar.

If the ovary is removed by horizontal incisions it will leave a less notable scar.
